Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations for health plan members in their homes and through virtual visits when needed.
The clinician will perform history reviews, physical evaluations and any ordered diagnostics, traveling locally and within licensed states as required, using an iPad-based workflow. Board-certified NPs/PAs with active licenses are preferred.
