Job Summary:

As a Social Worker Assistant, you will work closely with our healthcare professionals to provide exceptional care to our patients and residents.

• Assist social workers in conducting initial and on-going assessments of patients and residents to identify their physical, emotional, and social needs
• Develop and implement individualized treatment plans to meet the unique needs of each patient and resident
• Provide supportive counseling and therapy to patients and residents, as well as their families and caregivers
• Collaborate with healthcare teams to ensure comprehensive care and support
• Maintain accurate and detailed records of patient and resident care
• Participate in interdisciplinary team meetings to discuss patient and resident progress and develop strategies for improved outcomes

• BACHELORS degree in Social Work or a related field
• 2 years of experience in social work or a related field
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Proficiency in Electronic Medical Records (EMRs)
