Hartford Health Care, the largest and most integrated healthcare system in Connecticut, has a new opportunity available for a Congestive Heart Failure Nurse Practitioner to join our team cent’s Medical Center, in Bridgeport, CT. This position offers autonomous outpatient ambulatory clinic responsibilities with the chance to work with complex patients in a collaborative, provider-led environment.
Responsibilities- Autonomously manage a patient panel of recently discharged and chronic acute exacerbation patients in an outpatient ambulatory clinic setting.
- Remote monitoring of heart failure patients, management of LVAD patients, collaboration with cardiologists, and oversight and referral for comorbidities.
- Participate in per diem shifts (availability Monday–Friday, 8am–4pm) with highly competitive compensation.

Qualifications- Eligibility to be licensed as an Advanced Practice Registered Nurse (APRN) in Connecticut is required.
- Eligibility to be certified as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult Gerontology Primary Care Nurse Practitioner (AGPCNP), Acute Care Nurse Practitioner (ACNP), or Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) is required.
- Experience in practicing as an Advanced Practitioner is required. Congestive Heart Failure experience is highly preferred.
