Per Diem, Day shift (Monday-Friday 7a-3:30p)
- Implement and monitor patient care plans.
- Monitor, record, and communicate patient condition as appropriate.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Note and carry out physician and nursing orders.
- Assess and coordinate patient's discharge planning needs with the healthcare team.
- Demonstrate knowledge and skills to assess patients' physical, psychosocial, cultural, spiritual, and age-related needs.
- Delegate aspects of care to other nursing personnel consistent with their education and expertise.
- Participate in daily interdisciplinary patient rounds.
- Develop, implement, and revise individualized plans of care.
- Participate in activities for professional growth and development outside of patient care.
Education:
Associate degree required, BSN preferred.
Experience:
Cardiovascular nursing experience required.
Licenses &
Certifications:
Current Connecticut RN license. BLS certification by the American Heart Association.
