In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Bridgeport, CT

Signify Health, part of CVS Health®, seeks a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the person's health plan (e.g., diabetic eye exams, spirometry). Clinicians use an iPad and an intuitive clinical workflow; they are not required to prescribe medicine, order lab tests, or alter a patient’s current treatment regimen.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Be flexible to travel locally and within licensed states, as needed
• Conduct virtual visits as required
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE)

As an in‑home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present conditions such as household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in such spaces.

Base hourly rate or full‑time salary varies: $46.03 – $99.14. The exact offer depends on experience, education, geography, and other factors.
