Weekend LPN (Licensed Practical Nurse, Visting Nurse) - Home Care
As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes. Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability. Records daily progress in EMR. Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases. Communicates/conferences with primary nurse and other disciplines. Consults with physician regarding client needs and plan of care.
Qualifications:
- Minimum Education Requirement:
High School diploma or GED
- Successful completion of a CT state approved LPN certificate program - Minimum Licensure, Certification, Registration:
Current and valid LPN license
- Valid CT driver's license & a good driving history
Schedule:
Part-time (2 - 39 hours)
Shift Details:
Weekend LPN to work every Friday, Saturday, Sunday and Monday 8 AM- 4:30 PM
