LocationBridgeport, CT
Responsibilities- Autonomously manage a patient panel of recently discharged and chronic acute exacerbation patients in an outpatient ambulatory clinic setting.
- Perform remote monitoring of congestive heart failure patients.
- Manage LVAD patients and collaborate closely with cardiologists.
- Provide oversight and referrals for patient comorbidities.
- Work with complex patients in an autonomous ambulatory setting.
- Per diem position with available shifts Monday through Friday, 8am-4pm.
- Must be eligible for licensure as an Advanced Practice Registered Nurse (APRN) in Connecticut.
- Must hold or be eligible for certification as FNP, ANP, AGPCNP, ACNP, or AGACNP.
- Prior experience as an Advanced Practitioner is required; congestive heart failure experience is highly preferred.
Competitive hourly rates with weekly pay and malpractice included.
