Licensed Practical Nurse

Hartford Healthcare is seeking a Licensed Practical Nurse to provide compassionate care to residents in our assisted living community. The LPN will monitor resident health status, administer medications, perform treatments, and document care using EHR systems. You'll collaborate with RNs, physicians, and caregivers to support individualized care plans, promote safety, and respond to changes in condition. Responsibilities

• Provide direct nursing care to assisted living residents in accordance with care plans and physician orders
• Administer medications and treatments safely and accurately, following state scope of practice
• Monitor and document vital signs, symptoms, and changes in resident condition using EHR systems
• Assist with ADLs and support resident comfort, dignity, and independence
• Communicate effectively with residents, families, and the interdisciplinary care team
• Participate in care planning, shift reports, and quality/safety initiatives
• Maintain infection control, fall prevention, and safety standards
• Support orientation and guidance of aides and other support staff as needed
