About This Role
The Case Coordinator - RN is responsible for assessing and coordinating patient care along the continuum, focusing on discharge planning and addressing complex clinical needs. This role is ideal for experienced nurses who excel in collaboration and leadership within healthcare teams.
Highlights- Location: Bridgeport, CT
- Facility: St. Vincent’s Medical Center
- Schedule: Per Diem, Day shift
- Assess patient population in collaboration with the Health Care Team for appropriate level of care.
- Develop and implement discharge planning in collaboration with the Health Care Team.
- Collaborate daily with direct care providers to assess in-hospital stays and facilitate post-hospitalization services.
- Model quality patient/customer service by maintaining collaborative relationships with caregivers.
- Provide ongoing assessment and develop interventions and treatment strategies.
- Advocate for the needs and interests of patients, including system change for improved access to care.
- Educate patients, families, and professionals regarding disease prevention and treatment adherence.
- Maintain timely documentation of case management services.
- Identify discharge delays and develop countermeasures for potential barriers to discharge.
- Collaborate with providers to ensure appropriate patient placement throughout hospitalization.
Education:
- Completion of a formal Nursing program.
- Associate’s degree hires must obtain a Nursing Baccalaureate Degree (BSN) within 6 years of hire.
- Three years of acute-care setting, diversified clinical nursing experience.
- Case management, home care, and/or managed care experience strongly considered.
Certifications:
- Licensed Registered Nurse credentialed from the Connecticut Department of Public Health.
- Certified BLS Provider credentialed from the American Heart Association (AHA) or the American Red Cross preferred.
- Case Management Certification preferred.
