Overview

The Case Manager for the Enhanced Forensic Respite Bed (EFRB) Program plays a vital role in providing comprehensive case management services to individuals aged eighteen (18) or older with serious and persistent psychiatric disorders, or co‑occurring serious psychiatric disorders and substance use disorders, who are involved in active criminal court cases where competence to stand trial may be questioned. Under the supervision of the Clinical Coordinator, the Case Manager manages client care, coordinates admissions, facilitates transportation, connects clients to housing resources, and develops discharge plans to support successful community reintegration.

Responsibilities
  • Provide case management services in the person’s environment of choice, supporting everyday life based on their values and goals, and community integration.
  • Offer assessment, planning, advocacy, linking, support, and monitoring of activities focused on the individual’s needs, emphasizing basic survival needs such as housing, income, health, and community resources.
  • Assess and document significant medical, psychological, and social factors contributing to the client’s condition and inform other service providers in a timely manner to coordinate services.
  • Record adjustments and progress in each individual chart regarding finances, employment, housing, nutrition, clothing, and physical and mental health well‑being.
  • Record and monitor Residential Rehabilitation/Recovery plans and follow up to determine the quality and quantity of services provided.
  • Establish and maintain cooperative relationships with community resources, service providers, and other organizations.
  • Advocate on behalf of clients to obtain required services for successful community reintegration.
  • Redirect any client respectfully and appropriately as needed, offering supportive counseling and utilizing crisis intervention techniques and contingency plans based on the client’s needs and staff assessment.
  • Assist clients in maintaining a healthy environment, continuing a daily routine, and reconnecting with natural support systems in the community.
  • Identify appropriate aftercare plans and provide recommendations to the Jail Diversion team for court approval.
  • Arrange transportation as needed.
  • Perform other related duties as assigned by the supervisor.
Qualifications
  • Bachelor’s degree in social work, psychology, counseling, or a related field.
  • Experience working with individuals with serious and persistent psychiatric disorders, substance use disorders, and involvement in the criminal justice system.
  • Knowledge of case management principles and practices.
  • Strong communication, organizational, and problem‑solving skills.
  • Ability to work effectively with diverse populations and collaborate with multidisciplinary teams.
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality.
  • Ability to establish and maintain cooperative professional relationships.
  • Excellent communication skills and ability to project a professional image.
Certificates & Licenses
  • A valid Connecticut driver’s license may be required.
  • Incumbents may be required to secure professional certification and/or license.
Working Conditions

The position operates in a structured, community‑based setting staffed twenty‑four (24) hours per day, seven (7) days per week. Some evening and weekend hours may be required to accommodate program needs and participant schedules.
