Job Title

As a DDS Case Manager, you would be coordinating and performing case management for residential program participants and other participants who receive funding/service provided by Dept. of Developmental Services (DDS).

Monday thru Friday 8am-4pm

Responsibilities:

  • Provides case management to all individuals in the DDS IHS (In Home Supports) and other DDS funded individuals as assigned.
  • Assists participants in accessing all community resources and benefits available including financial, counseling and housing programs.
  • Develops, implements, and monitors annual service plans (ISP's) for all residential participants.
  • Assures compliance with all participants' funding sources.
  • Performs monthly record review for all assigned DDS participants to assure CARF compliance.
  • Communicates with participants' families, advocates, staff and other persons affiliated with programs on a regular basis to ensure customer satisfaction.
  • Coordinates medical appointments for all participants and ensures communication with nursing agencies and funding sources.
  • Ensures that all required documentation is completed to both DDS and CARF standards.
  • Monitors utilization rates to ensure that participant needs are met.
  • Schedules and supervises staff. Maintains accurate schedules for all participants, ensuring changes are made as they occur.
  • Participates in the On-Call rotation for all residential programs. On-Call is for one week on a rotation with other managers in the region and available after hours and on weekends for emergencies and staff call outs. Completes and submits the On-Call reports timely.
  • Actively participates in agency activities/trainings as required/requested by supervisor.
  • Attends all necessary team meetings and promotes open communication between Goodwill staff and outside funders, clinicians, etc.
  • Completes ongoing (Therap) case notes, meeting minutes, monthly reports and other paperwork as required to fully document progress and activities of assigned program participants.
  • Promotes full community and social integration of program participants.
  • Assures compliance with residential filing/record-keeping systems (Master Files).
  • Under the HIPAA Privacy Rule, this position has access to program participants' Protected Health Information (PHI).
  • Oversee and ensure compliance with HIPAA, CARF and all other relevant federal, state and local regulations.

Position Requirements:

  • Bachelor's degree in Human Services, Psychology or related field preferred.
  • Minimum 1 year of experience in direct care, case management or human services field.
  • An equivalent combination of education and experience may be considered.
  • Excellent verbal, written, organizational and administrative skills.
  • Solid computer application skills.
  • Current Connecticut driver's license and daily use of a reliable, insured vehicle.
  • Willingness to obtain and maintain medication certification, CPR and PMT certifications.
