The Start Program is a nationally recognized program that works with homeless and unstably-housed youth ages 18-24 throughout Connecticut. It combines short-term rental assistance with intensive, community-based case management services that use trauma-informed care and positive youth development to help participants exit homelessness and achieve long‑term stability.

The Case Manager2 provides home‑ and community‑based case management services to homeless young adults across the state. The role involves support, assessment, and partnership with participants to promote self‑sufficiency and housing stability.

• Deliver comprehensive case management services in clients’ homes and communities
• Conduct intake assessments
• Assist youth in locating available apartments for rent
• Work with youth to develop and implement individual service plans leading toward self‑sufficiency
• Serve as liaison with treatment providers and community resources
• Provide transportation to clients as needed
• Teach money management skills within caseload
• Teach employment readiness skills within caseload
• Act as liaison with landlords and property management staff to support continued tenancy
• Perform data entry and documentation
• Manage a caseload of 8‑12 young adults at any given time

• Associate’s Degree
• 2+ years relevant experience in direct service and case management
• Valid Connecticut Driver’s License
• Regular and predictable attendance is required

New employees will attend a mandatory two‑day New Hire Orientation at the Roscommon office in Middletown.
