The Connection's Start Program in Bridgeport, CT seeks a Case Manager 2 to deliver home- and community-based case management for homeless and unstably-housed youth 18–24 across Connecticut, supporting assessment and partnerships to promote self-sufficiency.
You will manage 8–12 youth, conduct intake assessments, assist with apartment searches, coordinate with providers, and document activities. An Associate’s Degree and 2+ years' experience, plus a CT driver's license, are required.
