The Work Place is seeking a Case Manager for the Platform to Employment Youth program in Bridgeport, CT. You will provide trauma-informed, individualized support to disconnected and at-risk youth aged 18-26, with travel to Norwalk, New Haven, Waterbury, and Hartford to engage participants and partners.
Based in Bridgeport, you will develop Employment Plans, coordinate wraparound services, and guide youth toward employment, education, or vocational training goals.
