Live Long Well Care is now accepting applications for Full Time and Part Time Home Health Aides to provide personal care and companionship for seniors. Day & Night positions are available as well as weekends. CHHA certification is required. Must be able to work some weekends, have a valid Driver’s License and a clean driving record. Services will be provided to various addresses within a 30‑mile radius of the branch address.

Must have an up‑to‑date smartphone for the electronic viewing and capture of Time and Attendance, Care Tasks, and Communication.

Position Summary

This position reports to the CSM (Client Services Manager) and/or designated supervisor. It includes the roles of Companion, Housekeeper, Homemaker, and Chore Worker.

Principal Duties and Responsibilities
  • Knowledgeable of, interprets, and supports the organization’s mission and policies.
  • Represents the company in a positive manner and fosters respect for clients and employees.
  • Provide general assistance and care to clients in accordance with the non‑personal care plan.
  • Plan meals, purchase food and supplies, and prepare and serve nutritious meals.
  • Maintain a clean, safe, and orderly environment for clients and families.
  • Perform housekeeping activities such as washing dishes, cleaning the kitchen after meals, launder clothes, and performing other household duties.
  • Provide companionship for clients as required.
  • Provide medication reminders.
  • Transport independent patients when directed by the supervisor.
  • Accurately and thoroughly document services provided, submit in a timely manner.
  • Immediately report any changes in client needs, condition, or incidents to the supervisor.
  • Establish and maintain effective communication and a professional relationship with clients/families and co‑workers.
  • Maintain and enhance skills through staff development programs.
  • Appear professional and comply with agency dress code.
  • Assignments are fulfilled with reliability and punctuality.
  • Speak and respond to client needs in a sensitive and timely manner.
  • Assist the supervisor by providing feedback on updating the care plan.
Professional Conduct & Development
  • Participate in Quality Assessment and Improvement activities as assigned.
  • Attend and participate in internal staff development programs and obtain continuing education as required.
  • Maintain and enhance clinical practice skills.
  • Perform other duties as assigned in response to client or facility needs.
  • Maintain strict confidentiality of patient and employee information.
  • Maintain strict confidentiality of agency proprietary and business information.
Other Duties and Responsibilities
  • Report for work on time and prepared to assume duties.
  • Adhere to company dress code.
  • Record work accurately and legibly.
  • Follow policy for reporting in sick or absent.
  • Schedule all time off with supervisory approval.
  • Varies work schedules to meet agency needs.
  • Adhere to company policy, procedure, and processes.
  • Respond to internal and external customers in a professional and friendly manner.
  • Offer assistance as needed or direct individual to appropriate resources.
  • Wears  in a visible location, if required.
  • Comply timely with all requirements related to risk management, safety, infection control, TB screening, security, and fire.
  • Provide a safe environment for clients, employees, and others.
  • Assume responsibility for all mandatory in‑services, certification, and/or licensure, competencies, etc.
Prerequisites
  • High school graduation or GED required.
  • Successfully completed a Home Health Aide program.
Direct Previous Experience
  • Two years’ experience in a similar position desired.
  • Two years’ experience working with the elderly desired.
Skills and Competencies
  • Fluently read, write, speak, and understand English.
  • Work with minimal or close supervision.
  • Conduct all business with a professional manner and high level of confidentiality.
