Now Hiring - RN Case Manager
Starting Salary: $90,000/Year
The RN Case Manager will be responsible for assisting the team with on‑call, case management, and visits to aid in coverage and call outs.
Responsibilities- Complete an initial assessment of patients and families to determine hospice care needs.
- Regularly re‑evaluate patients’ nursing needs.
- Use assessment data to determine care plan needs.
- Collaborate with the entire healthcare team, patients, and families to help direct the work of the team to provide continuous, well‑coordinated care.
- Counsel patients and families in meeting end‑of‑life needs.
- Initiate appropriate preventative and palliative nursing procedures.
- Communicate with the physician regarding patients’ needs and report any changes in the patients’ condition.
- Supervise the hospice aides.
Associate’s Degree in Nursing
Professional Experience- State‑certified RN license.
- Minimum of 2 years experience as a professional nurse; hospice experience preferred.
- Valid driver’s license and reliable transportation.
- Willingness to travel.
- Strong verbal and written communication skills.
