SummaryThe bilingual Family Support Case Worker will develop individual child case plans, track progress, and maintain complete case files, including information, education, referrals, outreach, advocacy and support to support the post‑release home placement of children. This position will maintain a network of contacts and referral resources to support sponsors and children. The position will assess safety and appropriate placement for children in the Juntos Home Study program with the goal to support family unification for immigrant children, family preservation, kinship care, and general child well‑being.
Dutiesand Responsibilities
- Provide information, education, referrals, outreach, advocacy, and support to ensure each child receives the services they require.
- Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practice.
- Develop individual case plans with particular attention to culture, language, and special circumstances.
- Maintain accurate and complete case files by updating database systems, submitting reports, and managing physical case files.
- Conduct calls, interviews, and home visits to assess safety and placement.
- Build a network of local pro‑bono legal resources, food and clothing banks, mental health services, and care providers to establish services for youth.
- Track individual progress via an online database.
- Coordinate and maintain communication with key stakeholders.
- Participate in regular training and communication sessions with on‑site staff and case workers, supervisor, and headquarters staff.
- Maintain confidentiality in all areas of the service population and program operations.
- Maintain professional and ethical standards of conduct outlined in the CFS employee handbook, including demonstrating respect for agency staff, the service population, and community members, and complying with required dress code at all times.
- Meet or exceed program productivity expectations on a regular and sustained basis.
- Ability to perform duties and services remotely from home.
- Perform other duties as assigned by supervisor.
- Bachelor of Social Work (BSW) degree or an equivalent degree in education, psychology, sociology, or other behavioral science fields.
- Bilingual in English and Spanish.
