Case Manager 124 - Full Time

Salary: $41,000 per year

Sign‑on Bonus: $2,000

• Working with and supporting family members and caregivers with an adult relative living with mental illness
• Provide case management services, including outreach, linkages, advocacy, referrals, and general support
• Assist to facilitate multi‑family psycho‑education and support groups, including researching ongoing psycho‑education topics, presentations
• Provides ongoing and communication with open caseload
• Ensure proper documentation in a timely and consistent manner

• Bachelor’s degree in SW, psychology or related degree with a minimum of 2 years as a Case Manager and 2 years working in the mental health field
• Some evening hours required
