Overview

The Adams County Health Department Nursing Division is comprised of the Nurse Family Partnership, Nurse Support Program, Harm Reduction, Sexual Health, Immunizations, Child Maltreatment and Fatality Prevention, and Care Coordinator for Children and Youth with Special Health Care Needs. The Division includes clinical support staff, medical assistants, harm reduction specialists, operations support, nurses, advanced practice nurses, and midwives.

Project Connects encompasses the Harm Reduction and Sexual Health programs, which work collaboratively to deliver comprehensive, accessible services across Adams County through brick‑and‑mortar locations, co‑located partner sites, mobile units, street‑based outreach, and community education and tabling efforts.

The Program Administrator provides strategic oversight for a multidisciplinary team—clinical and non‑clinical—to include nurse practitioners, registered nurses, medical assistants, harm reduction specialists, peer specialists, public health professionals, and leads a leadership team of program supervisors and managers. The role sets the program’s vision and direction, manages a portfolio of grant‑funded and locally funded initiatives, and collaborates closely with the finance team.

It requires developing and maintaining strong relationships with community partners to advocate for and promote awareness of the Project Connects impact.

Examples of Duties for Success

The position focuses on overall program and project implementation of assigned services. It involves the implementation of county‑wide clinical programs in coordination with community partners and other county departments. The position may support other public health services such as communicable disease, emergency response, and population health strategy.

This position performs a wide variety of professional duties and responsibilities, including leading the administration, coordination, and implementation of federal, state, and local grant programs. The Administrator ensures clinical performance and provision of nursing care in compliance with the State Board of Nursing and American Nurses Association standards and scope of nursing practice. Key components include collaborative planning with staff, multi‑tasking, problem‑solving, program development, fiscal tracking, independent project management, and reporting.

The role requires interpersonal relationships within the Health Department, Human Services, the healthcare community, partners, and the public, demanding professionalism, tact, good judgment, and communication skills.

  • Leads the planning, organizing, budgeting, project management, and compliance monitoring of assigned service area (Nurse Home Visitation, Sexual Health, or Immunizations).
  • Develops the required Quality Improvement Plan, Community Engagement Plan, Strategic Plan, Annual Report, and Performance Assessment in coordination with Performance Excellence, and in compliance with CMS regulations that identify community needs, especially those impacted by health inequities.
  • Develops program goals and objectives through research, data analysis, and public input.
  • Leads, facilitates, and manages programmatic community engagement and outreach.
  • Maintains clear communication with internal and external partners regarding nursing services and health department public health functions.
