Tobacco Cessation Navigator-Justice Involved Populations

The Tobacco Cessation Navigator-Justice Involved Populations (TCN-JIP) is part of the Health Access and Community Engagement unit (HACE) within the Adams County Health Department's (ACHD) Strategic Health Initiatives Division (SHI). The TCN-JIP is instrumental in supporting justice-involved populations by developing and delivering tobacco cessation education and promoting health education related to tobacco use with evidence-based health promotion activities, while supporting social need to improve outcomes.

This position contributes to tobacco and nicotine grant-funded prevention, policy, and systems change work that addresses social determinants of health (SDOH) impacting Adams County. This role leads and participates in the promotion of tobacco cessation education activities with justice-involved populations in the Adams County community through providing direct health education and increasing access to cessation resources and nicotine replacement therapy and supporting access to services that address SDOH to improve successful cessation attempts.

This position completes all grant monitoring and reporting using a critical and attention to detail.

Examples Of Duties For Success

Health Education And Navigation (60%): Develop and deliver smoking cessation group-education, ensuring culturally relevant materials and messaging. Support client needs by assessing SDOH and referring to appropriate services and supports.

  • Develop and lead tobacco cessation group education courses. Provide in-person and virtual group-based tobacco cessation education classes. Schedule routine and rotating classes that meet population need by providing courses in easily accessible community locations, providing support with meeting other SDOH needs of clients to ensure attendance and success of individuals. Manage all registrations, attendance, and health education materials for cessation classes.
  • Creates, adapts, and disseminates culturally relevant content for justice-involved populations.
  • Assess SDOH in all clients and provide referrals to supports and services, as appropriate.
  • Maintain appropriate records of attendance, health education provided, SDOH assessments, and referrals, ensuring compliance with HIPAA confidentiality.
  • Demonstrates leadership abilities, including educating, mentoring, training, and developing colleagues and community partners in program related areas.

Partner Engagement And Community Outreach (30%): Lead partner engagement and community outreach with identified transitional housing facilities and community-based locations within Adams County to execute population-specific, evidence-informed, and culturally relevant shared risk and protective factor health strategies that support tobacco and nicotine prevention and cessation activities.

  • Partner with up to 3 transitional housing facilities within Adams County for to provide routine information on smoking cessation education available to facility residents and clients.
  • Work closely with community-based and/or facility-based pharmacy partners to provide counseling and identify appropriate nicotine replacement therapies (NRT) for participating individuals. TCN-JIP will refer participants interested in NRT to pharmacy and/or health care partners to obtain recommendations on appropriate therapies. TCN-JIP will not provide recommendations or health advice.
  • Maintains active working knowledge of the social determinants of health; principles and practices of community outreach; principles of population-based health promotion and chronic-disease prevention; principles and practices of community engagement and developing trust and partnerships with justice-involved populations; motivational interviewing.
  • Exercises moderate decision-making latitude and works independently while consulting with staff at various levels, from entry level to executives and elected officials
