Advanced Practice Provider

Element Care, a nonprofit PACE organization, seeks an Advanced Practice Provider NP/PA to deliver comprehensive primary and specialty care to frail seniors living in the community. In this role, you will perform geriatric assessments, manage chronic and acute conditions, prescribe medications, and coordinate individualized care plans. You'll collaborate closely with an interdisciplinary team to support safe, independent living, provide family education, and ensure high-quality, patient-centered care that reflects dignity, respect, and cultural sensitivity.
