We are seeking a licensed Physical Therapist Assistant (PTA) to provide skilled physical therapy services to patients in their homes on an intermittent basis. Under the supervision of a Registered Physical Therapist, the PTA will play a key role in helping patients improve mobility, strength, and independence while remaining safely in their home environment.
As a PTA at No Place Like Home Health, you will use your clinical knowledge and compassionate approach to help patients live as independently as possible while receiving high-quality, personalized care.
Key Responsibilities- Provide physical therapy services as delegated by and under the supervision of a Physical Therapist (PT).
- Assist in patient evaluations, development of rehabilitative plans of care, and periodic re-evaluations.
- Perform routine strengthening and therapeutic exercises and assist patients with self-help and adaptive devices.
- Educate patients and family members on prescribed exercise programs and therapy techniques.
- Observe, document, and report patient responses to treatment and any changes in condition to the PT and physician.
- Identify and report potential adverse drug reactions, allergies, or contraindicated medications.
- Complete accurate clinical documentation, daily progress notes, and periodic summary reports within 48 hours for PT review.
- Participate in discharge planning and assist with completion of Physical Therapy Discharge Summaries.
- Communicate outcomes of home visits and physical findings to the PT in written form.
- Instruct home health aides, family members, and other care team personnel on appropriate therapy-related activities.
- Attend rehabilitation staff meetings and case conferences as required.
- Coordinate weekly visit schedules with the Supervisor or Clinical Manager.
- Maintain professional development through continuing education, workshops, and professional involvement.
- Perform other duties as assigned by the supervising PT, Registered Nurse Case Manager, or Nursing Supervisor.
- Ability to travel to and access patients’ homes, which may not be wheelchair accessible.
- Valid driver’s license and reliable transportation required.
- Ability to bend, stoop, reach, and assist with moving patient weight up to 250 pounds.
- Ability to lift and carry up to 30 pounds.
- Current Physical Therapist Assistant license in the State of Michigan
. - Graduate of a CAPTE-accredited Physical Therapist Assistant program
, or an equivalent program recognized by the American Physical Therapy Association. - Successful completion of the national Physical Therapist Assistant examination.
- Current CPR certification (online certifications not accepted).
