Aging and Dementia Services Coordinator – StoryPoint Group Position Summary
The Aging and Dementia Services Coordinator assists with the implementation, training, and facilitation of all Memory Care programs for Common Sail.
Required Experience for Assistant Director of Aging and Dementia Services- Bachelor’s degree in Gerontology, Recreational Therapy, or related field preferred.
- At least 3 years’ experience in Assisted Living and/or Memory Care preferred.
- Experience in leading and training teams.
- Ability to manage multiple projects and complete projects timely and accurately.
- Proven organizational and communication skills.
- Strong proficiency with Microsoft Office applications.

- Integrate and communicate the organization’s mission throughout all educational events.

- Assist with program Sustainability Excellence Reviews for quality measures and consistency.
- Participate in Memory Care Committee.
- Travel to communities (multi-state) to train and onboard Directors of Connection Points on Connection Points Memory Care Approach.
- Travel to communities (multi-state) to deliver training sessions to community leadership and team members, ensuring consistent implementation of the Connection Points Memory Care Approach.
- Provide ongoing education sessions as needed.
- Work closely with the Assistant Director of Aging and Dementia Services to develop and execute training plans for the successful roll‑out of the Connection Points Memory Care Approach.
- Demonstrates creativity and innovative thinking in challenging conventional views on aging and dementia in senior living.
- Report to the Director of Aging and Dementia Services, providing regular updates and progress reports.
- Perform other duties as assigned, contributing to the overall success of the department and organization.

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens.
The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Travel to different locations required.
