The Registered Nurse Case Manager is accountable for patients’ discharge plans and leads the achievement of clinical, financial, and resource utilization outcomes. This role coordinates care and services for defined patient populations throughout an acute care episode across the care continuum. The position supports Trinity Health Livingston (Brighton) and is a part‑time, benefited role scheduled for 24 hours per week, with orientation lasting 6–8 weeks from 8 a.m. to 5 p.m., Monday through Friday.

Position PurposeThe Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans and leads the achievement of desired clinical, financial, and resource utilization outcomes by coordinating care across the continuum of care.
What You Will Do- The RN will conduct a focused assessment of patient goals, biophysical, psychosocial, environmental, economic/financial, and discharge planning needs.
- They will review concurrently the appropriateness, intensity, and timeliness of level of care, diagnostic testing, and clinical procedures, quality and clinical risk issues, and documentation completeness.
- They will develop and document a discharge plan through collaboration with the patient, family, and interdisciplinary team, ensuring all activities to facilitate and coordinate the plan are implemented and modified as needed based on patients’ evolving needs.
- The RN will discuss the plan of care daily with physicians and care team members to affirm the need for acute care hospitalization while exploring the most suitable post‑acute care setting to meet patients’ needs and goals.
- Current Michigan Nursing License
- Graduation from an accredited nursing program (BSN required)
- Three (3) years recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager
- Case Management certification desirable; CMSA certification must be obtained within three years or be in process of obtaining
