School-Based Clinician

POSITION SUMMARY

The Bristol School-Based Integrated Health Care Clinician provides clinical and case management services for children in a school-based setting. The School-Based Integrated Health Care Clinician will foster positive working relationships with co-workers, clients and external key stakeholders. The School-Based Integrated Health Care Clinician will work in collaboration with teachers, administrators, and parents to provide high-quality mental health services to students in our school district.

The School-Based Integrated Health Care Clinician provides leadership, motivation and support to staff, manages the referrals, admission and discharge processes for clients, and ensures the integrity of the program model.

Master’s degree in Marriage and Family Therapy, Social Work, or Counseling, or Doctoral degree in Psychology.

Must hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut. Please note that you can apply prior to receiving the above credentials.

Full Time – This is a 10-month position.

The Integrated Healthcare Clinician works as part of a multidisciplinary team to efficiently and effectively meet the behavior, primary, and dental health needs of the client, including, but not limited to: providing behavioral health consultation to the primary care provider, assisting client in meeting medical treatment compliance, and referring client to other Wheeler services and/or other community services as needed.

Conducts program specific screening and evaluation, and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant, using behavioral health screening as directed, and are inclusive of behavioral health, primary care, and dental goals as indicated.

Demonstrates knowledge of psychopathology and primary modalities of psychotherapy, including individual, group and family. Correctly triages clinical emergencies, including suicide and danger to others. Uses best practices and evidence based practice models for psychotherapy where appropriate.

Makes effective use of topic oriented, focused, time-limited group therapy. Provides or refers to group when required.

Demonstrates knowledge of the use of psychotropic medications.

Completes clinical documentation in a timely manner.

Maintains and facilitates a minimum of 1 group during the school year, and utilizes evidence based practices (EBPs) to engage clients in model specific treatment targeted to address trauma.

Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.
