CARE COORDINATING

Position Summary:

We are looking for hourly caregivers for Bristol, Full-time/Part-time. Bilingual. Vehicle with valid Driver's License and auto insurance is strongly preferred. Must be dependable and reliable.

Provides personal care and homemaker/companion services under the direction of the Care Coordinator/Care Coordinating Supervisor.

The Person al care attendant is assigned to specific clients by Care Coordinator and performs services for clients as necessary to maintain their personal comfort.

Qualifications:
  • Be at least eighteen (18) years of age.
  • Minimum of six (6) months work experience in a supervised setting, preferably health care facility.
  • Experience providing care to elderly or disabled persons in the home setting.
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Good verbal, written, and interpersonal skills.
  • Reliable means of transportation to/from all assignments.
  • Valid driver’s license, registration, and insurance.
Essential Functions:
  • Performs household services essential to health care at home, including but not limited to:
    • Meal preparation/feeding
    • Laundry
    • Light housekeeping
    • Remind patients to take medications that are ordinarily self-administered under the direction and supervision of patient and/or family members
  • Performs personal care activities, including but not limited to:
    • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
  • Performs routine housekeeping duties including but not limited to:
    • Laundry
    • Changing of bed linens
    • Vacuuming/dusting
    • Grocery shopping, errands
    • Accompanies patient to scheduled appointments and social activities
  • Reports any observed or reported changes in the client's condition and/or needs to the Care Coordinator.
  • Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Care Coordinator.
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.
  • Communicates effectively with all members of the agency staff through verbal reports, participation in staff meetings, and team conferences, as requested.
  • Maintains confidentiality in all aspects of the job.
  • Successfully completes annual skills competency validation.
  • Attends in-service programs to meet compliance requirements.
Physical/Environmental Demands:
  • Lift/push/pull 50-75 lbs on a frequent basis.
  • Frequent stooping, twisting, turning, and bending.
  • Elevated risk of exposure to infectious processes.
  • Use of transfer and other household equipment.
