Job Summary:

• Plan, develop, and implement individualized nutritional plans for residents and patients
• Conduct assessments and monitor the nutritional status of residents and patients
• Collaborate with the healthcare team to develop and implement care plans
• Provide educational resources and support to residents, patients, and family members
• Conduct food demonstrations and nutrition counseling sessions
• Participate in Quality Improvement initiatives and contribute to the development of policies and procedures
• Maintain accurate and detailed records of patient care and nutrition plans
• Stay abreast of industry developments, trends, and best practices in nutrition and dietetics

• Bachelor's degree in Dietetics or related field
• Registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD) or eligible to become registered
• Current licensure in the state of Connecticut
• Minimum of 0 years of experience in a similar setting
• Strong communication and interpersonal skills
• Ability to work in a team environment and prioritize tasks effectively
