In the Home Health and Hospice environment—the Nurse Manager Hospice Care will guide a program recognized for high-quality care at a 150+ bed community hospital known as the #1 hospital in the state for preventing hospital-acquired infections, with a strong focus on patient safety, a dedicated and recently expanded emergency center, and a wide range of services including advanced robotic surgery;
hospice services deliver comprehensive, patient-centered end-of-life care grounded in comfort, dignity, and quality of life, evidenced by a 99.6% HIS quality score, and supported by 24/7 on-call support, personalized care plans, specialized dementia programs, and holistic support for families.
- Reporting to the Director of Home Health and Hospice within Nursing Services.
- Daily operations management.
- Staffing and scheduling.
- Mentoring and professional development of nursing staff.
- Budget stewardship.
- Regulatory readiness (CMS, Joint Commission).
- Development and implementation of clinical pathways and order sets.
- Interdisciplinary care coordination.
- Community partnership development.
- Performance improvement across HIS and patient experience measures.
