Key Responsibilities:
- Provide skilled nursing care to hospice and home health patients
- Conduct patient assessments, admissions, and routine visits
- Manage caseload and ensure continuity of care
- Develop and implement individualized care plans
- Coordinate with interdisciplinary teams
- Maintain accurate and timely documentation
- Meet productivity expectations of 24-30 visits per week
- Active RN license (PA)
- Hospice experience required
- Home Health experience preferred
- Experience with Home Care Home Base (HCHB) preferred
- Candidates without HCHB may be considered at a reduced rate during initial training
- Strong clinical and communication skills
- Valid driver's license and reliable transportation
- Pay Rate: $58/hour + Mileage reimbursement provided (IRS rate)
- Coverage Area:
Lower Bucks, Eastern Montgomery, Northeast Philadelphia (within ~60-mile radius) (Coverage may extend across service areas as needed) - Overtime must be pre-approved with written authorization
