DESCRIPTION OF WORK
This position focuses on supporting vulnerable older adults by managing a caseload and addressing complex situations. As a Aging Care Manager 2, you will perform the following duties:
- Caseload Management:
Maintain a caseload of vulnerable consumers and use independent judgment to handle routine and complex cases - Information Gathering:
Collect details from consumers, families, neighbors, and others to identify social, economic, emotional, health, or physical concerns - Investigation Work:
Investigate Reports of Need involving potential abuse, neglect, abandonment, or financial exploitation - Risk Evaluation:
Review gathered information to identify risks and apply regulations to determine substantiation - Protective Actions:
Provide protective interventions such as court petitions, emergency relocation, or referrals for community services
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.
- May be required to be on a standby rotation for off-hours coverage.
- Telework:

Required Experience, Training & Eligibility
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
