Aging Care Manager ; Local Government; Lehigh County Area Agency Aging

This position performs standard care management functions needed to support older adults by assessing their needs, developing care plans, and providing and coordinating services. Work supports consumers with identified social, economic, emotional, health, and physical problems as well as their families and caregivers. As an Aging Care Manager 2, you will perform the following duties:

• Care Planning: Prepare case histories and social assessment forms, obtain medical assessments and financial information to determine eligibility and develop a plan of care
• Case Documentation: Maintain accurate records of past and ongoing care; prepare memos, letters, summaries, and reports
• Consumer Support: Help consumers manage incomes, access long-term services and supports, and provide information to consumers and their families about available options

  Work closely with other social service agencies and professional staff, hospitals, clinics, courts, and community resources to coordinate care
• GUIDE Program Services: Provide comprehensive support and services, including home visits, to people living with dementia and their caregivers under the Guiding an Improved Dementia Experience (GUIDE) Model

• Full-time employment
• Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.

Minimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1;
  or
• Successful completion of the County Social Casework Intern program;
  or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field;
  or
• An equivalent combination of experience and training.
