As Clinical Pharmacist you will work as part ofa multi-disciplinary team to deliver a high- quality service to patients registered at Gloucester Road Medical Centre, Greenway Community Practice and Horfield Health Centre as part of our PCN (local network) Pharmacist team.
With your expertise in clinical medicines management, you will provide face to face structured medication reviews, management of medicines on transfer of care and systems for safer prescribing. If atrained independent prescriber, you will also prescribe for both routinere-authorisation and new treatment initiation.

About usWe are a supportive team and are looking for an enthusiastic individual to join us in providing excellent care for our patients. Team members should be flexible to work with both clinical and non clinical colleagues inhouse and to build relationships with community pharmacies and nursing and residential homes.
As a PCN we will (including this post) have six qualified pharmacists and two pharmacy technicians. This post is based at Gloucester Road Medical Centre and Greenway Community Practice but part of our PCN prescribing team with whom they will work closely.
Candidates that have not already been through the CPPE pathway will be supported to do so within their role. Finding the right fit for our team and service is most important.

Job responsibilitiesMain Functions ofthe role:
Working as part of amulti-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas
May involve leading onone or more long term condition (LTC) areas, as part of MDT approach to management of LTC across the team
Ensuring the integration of pharmacists across teams, attending groups and meetings as team rep where required
Lead on one or moreareas within the pharmacist team approach such as:
-
orepeat prescribing systems,
onursing and care homes
May involve supervising and supporting a pharmacy technician
May involve support and guidance of non- clinical Prescription Clerk team Leader
In an administrative role to process medication requests and reauthorisations.
To be aprescriber, or be working towards this, and work alongside the General Practitioners- contributing to clinical skill mix
Undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities
When clinically appropriate, to implement and action changes to the patients medical prescribing record as directed by primary, secondary or community care;working at all times within professional limits
Completing meds reconciliation for all new patient registrations
DEVELOPING CLINICAL PRACTICE(20%)
Development support and mentorship will be provided on a practice level, across the PCN and when available on a locality/CCG level. What is required will vary depending on the experience of the postholder.
You will be working alongside a team of pharmacists whose roles will be similar to your own andshared across the Primary Care Network (PCN).
This post MAY include an accredited training pathway for pharmacists (currently the CPPE Clinical Pharmacist training pathway), and in order to deliver the key responsibilities depending on the individuals training need
QUALITY IMPROVEMENT
To influence the prescribing behaviour of all members of the Practice MDT including GPs and practice nurses
Ensure prescribing and administration of medicines and disposal of medicinal waste follows appropriate legislation and health and safety policies.
To promote high quality, safe andcost effective prescribing as informed by evidence based practice and national and local guidance etc. To ensure prescribing conforms to relevant legislation
To initiate and complete a share of prescribing related audits
Providing clinical steeron medicines optimisation and quality improvement (including QOF)
Person Specification Experience- This position would suit a Clinical Pharmacist with or without Primary Care experience as training will be provided.
- Primary Care/GP Practice experience would be desirable but not a necessity as training will be provided.
