, managing typical medical problems to assist patients in transitioning from the hospital to home. Primary worksite is in

Call is provided
by a national telehealth group
with the
NP/PA available for back-up
and oversight of any transfers to the hospital.
Experience in Post-Acute/Long Term Care (PA/LTC) is preferred, but other experience is welcome.
