Shift Information 4 10 hour shifts 9a 7p during the week Monday through Friday

• Perform pre admission evaluations for the appropriateness of services
• Initiates the admission process by educating and informing patients families of its services
• Obtains informed consent
• Provides nursing assessment initial plan of care and interventions that optimize patient and family directed comfort and quality of life

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR Certification
• 1 year of Hospice experience preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients family members visitors agency personnel and the general public
• Must be knowledgeable of quality assessment and assurance procedures
