Salary Range: $23.00 To $26.50 Hourly
This position will be at HOPe's soon to be opened Brockton location - 142 Crescent St., Brockton MA
The Case Manager plays a vital role in supporting individuals with developmental and other disabilities by managing the person-centered planning process and ensuring quality, individualized service delivery. The Case Manager is responsible for coordinating, implementing, and monitoring Individual Support Plans (ISPs), maintaining accurate and timely documentation, and serving as the primary liaison between members, families/guardians, and program staff. This position focuses on service coordination, compliance, and communication—not direct supervision of staff.
The Case Manager collaborates closely with Site Managers and Lead Member Support Professionals (Lead MSPs) to ensure member goals are implemented through daily programming and activities.
- CPR/First Aid Certification. If not certified on hire, must be certified within the first 2 weeks.
- MAP Certification. If not certified on hire, must be certified within the first 3 months.
- Bachelor’s Degree in Human Services, Psychology, or related field preferred; equivalent experience considered.
- Valid Driver’s License and satisfactory driving record required.
Skills and Qualifications
- Knowledge of person-centered planning, ISP development, and DDS regulations.
- Excellent written and verbal communication skills.
- Strong organizational and time-management skills; ability to meet deadlines and manage multiple priorities.
- Minimum of two years’ experience in case management or service coordination within human services, health care, or a DDS-funded setting.
- Proficiency in Salesforce, HCSIS, and Microsoft/Google Workspace preferred.
