Job Duties/Responsibilities

Direct Care & Coordination of Services

  • Attend to individuals’ daily needs as well as foster increased independence and self-determination.
  • Teach socialization skills and activities of daily living including: household maintenance; self-care and hygiene; self-awareness and human development, menu planning, food shopping and preparation; money management; transportation and community orientation; and utilization of community resources.
  • Follow the Department of Public Health’s (DPH) Medication Administration Program (MAP) policies, which include, but are not limited to training requirements, medication administration, medication security, and corresponding documentation including in BAMSI’s Electronic Health Care Record.
  • Ensure individuals’ nutritional needs are met based on personal preferences and according to prescribed diet. Provide support with preparation and eating as necessary.
  • Promote physical health and wellness routines.
  • Follow individualized protocols for medical and safety needs to ensure proper steps for prevention, maintenance, follow up, and reporting take place as required.
  • Use supportive and adaptive, health-related protections/equipment as prescribed. Assist individuals as needed and required with cleaning of equipment as directed.
  • Support individuals served with achieving the goals and objectives outlined in service/support plans.
  • Use approved crisis management techniques to deescalate individuals and prevent injury.
  • Positive Behavior Supports & Community Connections

  • Utilize Positive Behavior Supports (PBS) practices, to create a thoughtful and supportive environment by ensuring activities, plans, and routines reflect the preferences, needs, and goals of individuals.
  • Use integrated strategies to support personal strengths and assets of each individual served.
  • Support individuals to make decisions across all environments to the greatest level of each person’s ability.
  • Role model behavioral awareness through demonstrating and encouraging pro-social behaviors and maintaining appropriate professional boundaries with individuals served.
  • Learn and utilize communication tools which include but are not limited to email, tablets, and letter boards, taking into consideration the method most suited to individual needs. Communicate in the preferred method and language of those you provide support.
  • Promote healthy living and relationship building with all people, supported through individualized social, spiritual, and recreational opportunities offered in membership, access, and integration in their communities.
  • Assist with access to community, including transportation as needed, to individuals served to access community. Examples include but are not limited to transporting individuals served to and from day programs, work, medical appointments, to family and friends visits, and other recreation and leisure activities as assigned.
  • Communication, Documentation & Teamwork

  • Ensure person served safety and compliance with program standards for staff coverage and reportable incidents, including assistance to the administrator on call whenever possible as assigned to provide support/coverage needs to other sites or locations as identified and required for immediate needs of person served.
  • Follow funder and agency protocol for reporting significant incidents and alleged abuse and/or neglect while fulfilling legal responsibility as a mandated reporter.
  • Work with community physicians and staff to develop and implement effective medical treatment plans.
  • Assist with admissions and discharges to and from hospitals and rehabilitation centers.
