Position Overview
Harbor Health Services is seeking a passionate Dietitian to deliver high-quality nutrition care to elders and support their health, independence, and dignity at our Elder Service Plan/PACE Program in Brockton.

Salary Range: $62,500 - $93,800 annually
Starting Date: ASAP
Sign‑on Bonus: $2,500 (half on hire, half at 90 days in good standing; former employees of Harbor Health are not eligible)

Responsibilities- Perform and document timely initial and annual nutrition assessments, including diet order entry, height/weight, lab results, dietary history, preferences, medical conditions and medications affecting nutrition status.
- Develop individualized nutrition plans and ensure effective communication of plans to participants, IDT, Dietary Aides, family members and other caregivers.
- Communicate concerns or issues arising from Nutrition Assessments to IDT.
- Provide nutrition education materials and one‑on‑one guidance to participants, families and caregivers for implementation of the Nutrition Plan.
- Recommend and ensure provision of nutrition supplements when appropriate, monitoring and documenting participant use.
- Arrange for home‑delivered meals and specialized meals or supplements for participants as needed.
- Assist the Nutrition Manager in ensuring that Dietary Aides perform necessary food handling and maintenance duties, maintaining proper temperatures, equipment, logs, labeling and compliance with OSHA, DPH, PACE and CMS regulations.
- Use objective information from EMR combined with subjective food intake information to complete comprehensive and accurate assessments.
- Assist in presenting Annual Safety training for ESP staff on food handling, temperatures, feeding of participants and kitchen sanitation to Dietary Aides, Center Aides and Home Health Aides.
- Provide functional supervision to Dietary Aides during Adult Day Health Center kitchen operations.
- Bachelor’s degree in Food and Nutrition or Dietetics (Master’s preferred).
- Current Massachusetts Registered Dietitian license and BLS (Basic Life Support) Certification.
- Must obtain Serve Safe certification (Food Handlers or Serve Safe Manager) within 3 months of employment.
- Must obtain Mass Allergen Certificate within 3 months of employment.
- Intermediate skills with Word, Excel and Outlook.
- One year of professional experience providing nutrition counseling and assessments.
- One year experience in geriatrics required.
- Excellent written and oral communication skills.
- Ability to travel to multiple patient homes and sites in a timely manner.
