Licensed Practical Nurse

Community Health Programs (CHP) is actively seeking a dedicated Licensed Practical Nurse (LPN) to join our growing network of health centers. At CHP, we prioritize a holistic approach to health by integrating medical, dental, and behavioral services to fully support the wellness of the communities in Berkshire County. As an LPN at CHP, you will play a critical role in delivering high-quality, community-based care in a supportive and collaborative environment.

• Administer nursing care to ill, injured, or disabled patients within the guidelines set by the facility.
• Monitor patient health and maintain accurate records of vital signs and patients' health progress.
• Assist in the implementation of patient care plans set forth by registered nurses or doctors.
• Prepare and administer injections and prescribed medications as directed by the physician.
• Perform routine lab tests and therapeutic services.
• Educate patients and their families on health maintenance and disease prevention.
• Collaborate with a multidisciplinary team of healthcare providers to enhance patient care.

• Proficient in administering medications and immunizations.
• Strong communication and interpersonal skills.
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.).
• Excellent organizational and time management skills.
• Ability to make decisions quickly and in high-pressure situations.
• Valid LPN license to practice in the state of Massachusetts is required.
• CPR certified or ability to obtain certification shortly after employment begins.
