Summary
Registered Nurse (Community Care Coordinator) position located in the Community Care Services at the Leeds - MA location. Full‑time, 40 hours per week.
Qualifications- English Language Proficiency – written and spoken.
- Graduate of a professional nursing school approved by the appropriate accrediting agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
- Or graduate of a master's level bridge program in nursing who has completed coursework equivalent to a bachelor's level degree in Nursing.
- Upon achievement of a state license, the individual may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
- In cases of graduates of foreign schools of professional nursing – possession of a current, full, active, and unrestricted registration meets the requirement for graduation from an approved school of professional nursing to warrant appointment as a nurse who has completed an associated degree/entry–level nursing education program.
- Credit for foreign nursing education higher than an associate degree/entry level requires formal degree equivalency validation from a recognized equivalency evaluation accepted by VA, such as International Consultants of Delaware (ICD).
- Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory, or Commonwealth (e.g., Puerto Rico) or the District of Columbia.
- Graduate Nurse Technician (GNT) Exception:
Candidates who otherwise meet the basic education requirements but do not possess the required licensure may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120‑day temporary appointment while actively pursuing licensure (may be extended up to two years on a case‑by‑case basis). - Grade determinations are based on scope, education, and dimension criteria detailed in the VA assessment.
- Preferred Experience:
VA community care experience, care coordination and/or case management experience, outpatient experience. - Physical Requirements:
All applicants must be physically able to satisfactorily perform the duties of the position. - A pre‑placement physical evaluation shall be completed prior to appointment.
- Responsible for all elements of the nursing process when providing and/or supervising patient care.
- Assesses, plans, implements, and evaluates the effectiveness of the veteran’s care through telephone interaction.
- Supports delivery of high‑quality, timely care to the veteran receiving services in the community.
- Coordinates care focused on patient education, self‑management, and customer satisfaction throughout the continuum of care.
- Follows procedures per established policies and guidelines.
- Collaborates with members of the interdisciplinary team to influence care outcomes.
- Participates actively in the unit/program‑level quality improvement process and initiatives, and customer service programs.
- Works Monday through Friday, 8:00 a.m. to 4:30 p.m. (telework not authorized; position is not virtual).
