• Assist with health assessments and participant wellness monitoring; support the development and implementation of individualized care plans.
• Maintain accurate nursing documentation, medication administration records, and physician orders.
• Promote health education, wellness initiatives, and preventative care practices.
• Collaborate with Registered Nurses, physicians, specialists, families, and interdisciplinary team members to ensure coordinated care.
• Participate in care planning meetings and contribute to positive participant outcomes.
• Help maintain a safe, healthy, and person-centered environment for all participants.

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Current Massachusetts LPN license required.
• Current CPR and First Aid certification.
• One year of nursing experience preferred, particularly with medically complex adults, seniors, or individuals with disabilities (preferred but not required).
• Strong communication, organizational, and interpersonal skills.
• Bilingual Cape Verdean or Portuguese highly preferred.
