About the Role:

• Provide direct patient care including monitoring vital signs, administering medications, and wound care
• Assist in developing and implementing individualized patient care plans
• Collaborate with RNs, physicians, and interdisciplinary care teams to ensure quality outcomes
• Document patient assessments, treatments, and progress accurately in medical records
• Educate patients and families on care procedures, medications, and discharge instructions
• Respond promptly to patient needs and changes in condition
• Maintain compliance with all state, federal, and facility healthcare regulations

• Active LPN license in the state of Massachusetts (required)
• Graduate of an accredited Licensed Practical Nursing program
• Current BLS/CPR certification
• Previous clinical experience in a healthcare setting preferred
• Strong communication, organizational, and critical thinking skills
• Ability to work independently and as part of a collaborative care team
• Comfortable using digital platforms; familiarity with the Oncoll App is a plus
