Job Description Position Summary

The Young Adult Housing Stabilization Case Manager supports young adults (ages 18–24) as they transition into safe, stable, and independent housing. This role blends intensive case management, skill‑building, advocacy, and long‑term stabilization planning. The Case Manager works closely with community partners, landlords, and internal teams to ensure each participant has the resources, support, and guidance needed to maintain housing and move toward self‑sufficiency.

Key Responsibilities
  • Provide ongoing stabilization services to young adults placed in permanent or transitional housing programs.
  • Develop individualized service plans focusing on housing retention, income growth, education, and wellness.
  • Conduct regular home visits to assess stability, identify risks, and reinforce independent living skills.
Case Management & Coaching
  • Maintain a manageable caseload of young adults, offering consistent, trauma‑informed support.
  • Assist participants in accessing benefits, employment opportunities, education programs, and community resources.
  • Support young adults in developing life skills such as budgeting, communication, conflict resolution, and household management.
Documentation & Compliance
  • Complete eligibility assessments, service plans, progress notes, and required documentation in accordance with program and funder requirements.
  • Track participant progress and housing outcomes, ensuring accuracy and timeliness in data systems.
  • Administer short‑term rental assistance using a progressive engagement framework, ensuring participants regain self‑sufficiency and rental responsibility before exiting the program.
  • Partner with landlords, property managers, and community providers to support housing stability.
  • Advocate for participants in navigating social services, behavioral health supports, employment systems, and educational institutions.
  • Participate in team meetings, case conferences, and interdisciplinary collaborations.
Crisis Prevention & Intervention
  • Identify early warning signs of housing instability and develop strategies to prevent eviction or displacement.
  • Provide crisis support and coordinate interventions that promote safety, stability, and continuity of care.
Youth-Centered Engagement
  • Build trusting relationships with young adults while recognizing the developmental needs and autonomy of emerging adults.
  • Foster an empowering, judgment‑free environment that encourages skill‑building, confidence, and long‑term goal achievement.
  • Facilitate groups or workshops related to life skills, employment readiness, wellness, or community engagement when needed.
Qualifications
  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field (or equivalent experience).
  • At least 3 years Experience working with young adults, transitional‑aged youth, or individuals facing housing instability preferred.
  • Strong understanding of trauma‑informed care, youth development, and harm reduction principles.
  • Ability to manage time effectively, maintain detailed documentation, and work both independently and as part of a team.
  • Comfort responding to crises and de‑escalating challenging situations.
  • Valid driver’s license and reliable transportation preferred.

Combination of office work, community-based visits, and home‑based support.

Some evening flexibility may be required based on participant needs.
