Old Colony Elder Services (OCES) seeks a One Care Plan (OCP) Advocate to empower individuals with disabilities in accessing Long Term Services and Supports (LTSS). In this role, you will work closely with consumers to assess needs, develop care plans, and provide advocacy.
Qualified candidates should have a Bachelor's degree in social work or a related field, a valid Social Work License, and at least two years of relevant experience. The position offers a flexible 35-hour work week and requires occasional travel.
Salary is $25.40 per hour.
