Responsibilities
The Discharge Planner functions as a member of the interdisciplinary treatment team with primary responsibilities for discharge planning. Acts as a liaison with referral sources and other departments within the hospital.

- Education:
Bachelor's degree in social services or related field required. - Behavioral health experience is a plus.
