Position Summary

YourADH (formally Vitra Adult Day Health) is seeking an experienced and compassionate Adult Day Health Social Worker to provide professional supervision, clinical consultation, and guidance for social work services within our Adult Day Health (ADH) program.

The Adult Day Health Social Worker provides ongoing professional oversight to promote high-quality, ethical, culturally responsive, and client-centered social work services. This role serves as a clinical and professional resource to the Case Manager regarding psychosocial assessments, care planning, referrals, documentation, complex client and family situations, community resources, and coordination with the interdisciplinary team.

The Social Worker will provide approximately one hour of professional supervision per week and consultation as needed. This position provides professional oversight, consultation, and guidance to support effective service delivery and regulatory compliance.

Key Responsibilities
  • Provide regular professional supervision and consultation to the ADH Case Manager, including approximately one hour of supervision per week.
  • Provide professional guidance related to psychosocial assessments, care plans, client-centered goals, interventions, referrals, and follow-up.
  • Consult with the Case Manager regarding complex, high-risk, or challenging client and family situations and recommend appropriate interventions.
  • Review social work documentation and practices to promote accuracy, quality, confidentiality, and compliance with organizational and regulatory requirements.
  • Support the Case Manager in coordinating social work services with clients, families, caregivers, healthcare providers, community agencies, and interdisciplinary team members.
  • Provide guidance regarding community resources, healthcare services, social service agencies, benefits, and other referrals that address client needs.
  • Support effective case management and care coordination for older adults, individuals with disabilities, and other vulnerable populations.
  • Provide professional guidance regarding applicable Massachusetts Adult Day Health, Department of Public Health (DPH), Mass Health, and other regulatory requirements.
  • Support the Case Manager and ADH program in maintaining complete and appropriate documentation, regulatory compliance, and audit readiness.
  • Identify and communicate significant psychosocial concerns, safety risks, compliance issues, and recommendations to the ADH Program Director and appropriate members of the interdisciplinary team.
  • Promote ethical social work practice, client advocacy, confidentiality, dignity, and culturally responsive care.
  • Serve as a professional resource to the ADH team regarding psychosocial needs, social determinants of health, community resources, and care coordination.
  • Collaborate effectively with nursing, medical, rehabilitation, behavioral health, case management, and other interdisciplinary team members.
  • Maintain professional knowledge of relevant Massachusetts regulations, healthcare programs, community resources, and social work practices.
Required Qualifications
  • Master of Social Work (MSW) degree preferred.
  • Licensed Clinical Social Worker (LICSW) required.
  • Demonstrated knowledge of professional social work practices, including psychosocial assessment, care planning, case management, referrals, advocacy, and client support.
  • Experience working with older adults, individuals with disabilities, medically complex individuals, or other vulnerable populations preferred.
  • Previous experience in social work supervision, clinical consultation, case management, or professional oversight preferred.
  • Knowledge of community-based healthcare and social service systems.
  • Knowledge of Massachusetts Adult Day Health requirements and applicable healthcare and social service regulations.
  • Strong understanding of confidentiality, professional ethics, documentation standards, and client rights.
  • Excellent written and verbal communication skills.
  • Strong interpersonal, organizational, critical-thinking, and problem-solving skills.
  • Ability to provide constructive professional feedback and guidance to Case Management staff.
  • Ability to exercise sound professional judgment when addressing complex client and family situations.
  • Ability to maintain confidentiality and handle sensitive client information appropriately.
