Salary: $24.00 - $26.00 per hour

Licensed Practical / Vocational Nurse – LPN/LVN. Provide skilled nursing care in patients’ homes, supporting both adults and children. Deliver compassionate, evidence‑based care while coordinating with clinicians and families.

• One‑to‑one patient care in a home environment.
• Execute physicians’ prescribed plans of care and document care in the electronic system of record.
• Administer prescribed medication, treatments, and therapies.
• Conduct patient assessments and coordinate care with other providers.
• Promote health, educate patients and families, and provide training as needed.

• Active, unencumbered LPN/LVN license in the state where practice occurs.
• Compact license transferred to state of residence within 90 days.
• Current CPR certification with hands‑on component.
• TB skin test within last 12 months.
• Minimum 6 months of hands‑on nursing experience preferred.
• Reliable transportation.
• Willingness to provide in‑home nursing care to infants, children, and adolescents.
