Gentiva Health Services, Inc. is seeking a dedicated After hours RN Care Manager to provide high-quality hospice care outside standard business hours. This position ensures continuity of care and clinical management for patients and families during nights, weekends, and on-call hours.
The ideal candidate will assess and coordinate patient needs, provide in-person patient care, and support interdisciplinary communication. Candidates must have a current RN license and at least one year of relevant experience.
