• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living
• Collaborate with interdisciplinary teams to develop and implement patient care plans
• Conduct vital signs, monitor patient conditions, and report changes to nursing staff
• Participate in providing education and support to patients and families
• Maintain accurate and up-to-date patient records
• Assist with scheduling and documenting doctor's orders and treatments

• Current LPN license in the State of New York
