Field Operations Specialist; Mobile Bronx​/Westchester

Elder Serve Health is a mission-driven, nonprofit managed care organization serving the New York metropolitan area. Elder Serve provides long-term services and supports to more than 20,000 older adults and adults with disabilities across New York City, as well as Westchester, Nassau, and Suffolk Counties. Elder Serve primarily serves a frail and elderly membership who is predominantly dual-eligible and enrolled in Medicaid managed long-term care, Medicare-only and integrated Medicaid-Medicare plans.

The Care Source and Elder Serve affiliation addresses these critical long-term care needs to help members be as healthy and independent as possible.

• Schedule and travel to members' homes to conduct in-home care management visits.
• Support the provision of care plans by assisting Nurse Care Managers with member interactions and take authorizations from Nurse Care Manager for processing.
• Answer incoming calls, place outgoing calls as requested and/or review member care management survey results identifying issues requiring follow-up.
• Coordinate services, take requests for services, assist Nurse Care Manager in placing authorized services and speaking to members.
• Assist members in navigating the Health Plans process by phone and in person visits.
• Establish and maintain relationships acting as the liaison among members, clinical staff and external entities.
• Perform any other job related duties as requested.

• High School or GED required
• Associates degree preferred
• Medical terminology experience is strongly preferred
• Experience using multiple languages in a professional capacity required

• Proficient in Microsoft Office Suite to include Word, Excel and Power Point
• Fluency (speaking, reading, writing) in English and Spanish
• Strong written and verbal communication skills
• Effective listening and critical thinking skills
• Strong interpersonal skills and high level of professionalism
• Ability to define problems, collect information and establish facts
• Ability to work independently and within a team environment
• Ability to develop, prioritize and accomplish goals
• Knowledge of applicable Federal, State and Local regulations as it applied to required job functions

• Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver's license record check. If the driver's license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminated
• To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. Care Source requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 - March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Compensation Range:
$41,200.00 - $66,000.00 Care Source takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
