• LMSW: $70,000 - $80,000
• LCSW: $80,000 - $90,000+ (based on experience and qualifications)

We are seeking a compassionate and clinically skilled Licensed Social Worker (LMSW or LCSW) to join our integrated behavioral health team. This hybrid position offers the opportunity to provide high-quality behavioral health services while collaborating closely with primary care providers, psychiatrists, nurse practitioners, care managers, and other healthcare professionals.

The ideal candidate will be passionate about improving patient outcomes through evidence-based interventions, behavioral health assessments, crisis intervention, care coordination, and psychotherapy services.

• Conduct comprehensive behavioral health and biopsychosocial assessments using evidence-based screening tools.
• Develop, implement, and monitor individualized treatment plans.
• Provide psychotherapy and behavioral health interventions utilizing evidence-based approaches including:
• Cognitive Behavioral Therapy (CBT)
• Motivational Interviewing (MI)
• Behavioral Activation
• Interpersonal Psychotherapy (IPT)
• Crisis Intervention
• Conduct risk assessments and provide crisis intervention services when needed.
• Participate in collaborative care team meetings and provide treatment recommendations.
• Monitor patient progress and adjust treatment plans accordingly.
• Coordinate care with primary care providers, psychiatrists, nurse practitioners, and community resources.
• Facilitate referrals to behavioral health, medical, and community-based support services.
• Engage patients through outreach efforts and promote adherence to treatment plans.
• Maintain timely, accurate, and compliant clinical documentation within the electronic health record (EHR).
• Participate in clinical supervision, team meetings, case conferences, and quality improvement initiatives.

• Master of Social Work (MSW) from an accredited institution.
• Active New York LMSW or LCSW license.
• Bilingual English/Spanish required for LMSW and preferred for LCSW.
• Experience conducting behavioral health assessments and psychosocial evaluations.
• Experience developing and implementing treatment plans.
• Knowledge of DSM-5 diagnostic criteria.
• Experience with crisis intervention and risk assessment.
• Proficiency with electronic health records (EHR).
• Strong communication, documentation, and interdisciplinary collaboration skills.

• Experience providing psychotherapy in outpatient behavioral health settings.
• Integrated behavioral health or collaborative care experience.
• Experience working with diverse and underserved populations.
